Job Description
If you do not have a Bachelor's Degree, the payrate is $18.00 per hour. If you have a Bachelor's degree, the payrate is $20.00 per hour. The rate will increase based upon experience.
REQUIREMENTS
Bachelor's degree PA Driver's License All Required Clearnaces (PA Child Abuse, FBI FIngerprints, Criminal Background Check) Physical (with a Tuberculosis Test) Available to work evenings Partially flexible schedule to meet theneeds of the child & the familySUPERVISION
➢ Supervised by the Foster Care Supervisor and Foster Care Assistant Supervisor ➢ Daily operations and guidance of the agency overseen by Administration ➢ Attends and completes training, supervision sessions, and employee evaluations, as required by department superiors or administrationDESCRIPTION OF DUTIES
➢ Participates in on-call rotation ➢ Accepts cases delegated by department superiors ➢ Gather information concerning each child's biological family, medical, mental health treatment, behavioral health, educational, legal, social, recreation, employment, independent living, employment, safety, and mood ➢ Assist child in identifying specific strengths about skill, knowledge, ability, and interests as well as supports for improved psychosocial functioning ➢ Develop an Individual Service Plan (ISP) with children, foster parent(s), biological parent(s), case workers, and others to include in activities necessary to enable the child to live as an integral part of the community and the child's family Identify specific goals which the child desires to achieve Formulate specific objectives to focus on completion of the goals Identify resources available to the child that may help meet desired objectives Identify means to monitor and update progress toward achievement of goals and objectives ISP should be reflective of the child's family of origin ➢ Make aggressive and creative attempts to help the child gain access to resources and required services identified in the service plan Engage natural resources and supports (i.e., family, neighbors, support groups) to serve as the primary mode of assistance, where possible Utilize existing community services or social service agencies to the extent not met by natural resources Make referrals to community mental and behavioral health services Aid foster and biological families in identifying, accessing, and learning to use community resources ➢ Ensure the ongoing involvement of the child in a supportive network of natural and community resources to the degree necessary to affect continued personal/social growth or rehabilitation Facilitate acceptance and attendance in programs and activities by frequent interaction with service providers Participate in on-site activities, including attending appointments with the foster family, family of origin, doctors, social workers, therapists, employers, teachers, and other significant people involved with the child for the purpose of assisting the child in achieving aspects of the service plan Expected to maintain face-to-face contact with any child who is receiving inpatient services at a community psychiatric unit for purposes of planning of the child's discharge ➢ Provide support and problem resolution assistance through direct efforts asCRISIS SUPPORT
staff Remain accessible to all children and foster families on the caseload for problematic events Serve as the primary contact and support in the event of an emergency and directly assist in the child and foster family in resolving crises Link families to appropriate resources to assist with the diminishment or negation of the problem Provide options to assist in learning to deal with reoccurring problems ➢ Participate in meetings/workshops with other agencies for the purpose of resource sharing, program planning, and problem solving ➢ Complete all paperwork requirements of the agency, county, and state authorities to include: Maintaining organized, individual client record of all documents pertaining to the services provided Referral information and forms Verification of medical, dental, and vision care Verification of attendance and compliance with mental and behavioral health services Priority Assessments Progress/Court Reports Appropriate Contact Logs All consent and authorization forms Service plans and updates Termination Summary, when applicable Other required recordsQUALIFICATIONS
➢ A bachelor's degree from an accredited college or university OR High School Diploma or Equivalent with adequate work and life experience ➢ Valid PA Driver's License ➢ Be free of communicable disease and be capable of performing the responsibilities of the job, as verified by an annual medical appraisal ➢ Hold current clearances from Childline, PA State Police, and FBI Fingerprint (or as required by OCYF) ➢ Computer literacy (Word, PowerPoint, Excel) ➢ Excellent communication skills, both written and verbal ➢ Ability to perform all aspects of the position requiresPay:
