Job Description

Overview Join our dynamic team as a Service Coordinator and become a vital catalyst for empowering individuals with developmental disabilities to lead fulfilling, independent lives. In this energetic role, you will coordinate and oversee service delivery, foster meaningful relationships with clients and their families, and ensure the seamless operation of programs designed to promote social, behavioral, and community integration. Your proactive approach will help create a supportive environment where every individual's unique needs are met with compassion and professionalism. Responsibilities Manage and coordinate individualized service plans for clients, ensuring all program goals are met effectively and efficiently. Develop an individualized plan that promotes personal growth, social engagement, health improvements, and supports each individual that we serve to have the services and supports that they require to have a better life. Build strong relationships with clients, families, community partners, and service providers to facilitate collaboration and ongoing support. Oversee administrative tasks such as documentation, progress reports, budgeting, and compliance with regulatory standards. Monitor program outcomes through data collection and analysis to continuously improve service quality and client satisfaction. Assist in managing program budgets, resource allocation, and scheduling to ensure operational excellence. Skills Previous Case Management experience a plus Strong working knowledge of behavioral health principles and social work practices related to this population. Exceptional relationship management skills to foster trust and collaboration among clients, families, staff, and community partners. Solid administrative experience including documentation, record keeping, budgeting, and compliance oversight. Must have exceptional time management skills, be self-directed, and have a strong work ethic. Familiarity with program management tools and techniques to ensure smooth operation of services. Pay:
$22.00 - $22.11 per hour
