Job Description

Join our dedicated healthcare team as a Shared Care Caregiver CNA/PCA, where your compassionate nature and clinical skills will make a meaningful difference in patients' lives. This dynamic role offers the opportunity to provide essential personal and medical care across various settings, including long-term care facilities, assisted living communities, hospitals, and home care environments. Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts and other assistive devices Monitor vital signs, document medical records accurately using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems, and report any changes in patient condition promptly Medication reminder following prescribed protocols, ensuring compliance with HIPAA regulations and aseptic techniques Support patients with specialized needs including dementia, Alzheimer's disease, memory care, behavioral health issues, and developmental disabilities Perform catheter care, wound care, and other medical-surgical procedures under supervision or as trained Collaborate with interdisciplinary teams in acute care, post-acute settings, or outpatient clinics to ensure comprehensive patient care plans are followed Maintain a safe environment by adhering to infection control standards and utilizing proper safety equipment such as Hoyer lifts and PPE (Personal Protective Equipment) Current CNA (Certified Nursing Assistant) or PCA (Personal Care Assistant) certification required; active licensure preferred Experience working in diverse healthcare settings including hospitals (Level I Trauma Center experience preferred), nursing homes, assisted living facilities, or home care environments Knowledge of medical terminology, physiology, anatomy, and aseptic techniques necessary for safe patient care Familiarity with EMR/EHR systems such as Cerner or eClinicalWorks for accurate documentation Demonstrated ability to work with vulnerable populations including seniors, children, individuals with disabilities or behavioral health needs Critical care experience such as ICU or emergency medicine is highly valued Strong communication skills and a compassionate attitude toward patient service and caregiving responsibilities Join us in delivering vital healthcare services that improve lives every day!
