Key Responsibilities:

Collaborate with interdisciplinary teams to assess patient needs and develop individualized discharge plans. Qualifications:
Bachelor's degree in Nursing, Social Work, Healthcare Administration, or a related field. Proven experience in discharge planning or case management, ideally in a healthcare or rehabilitation setting. Strong understanding of healthcare regulations and community resources.
