Full-time Social Services Director/ Discharge Planner Position Summary :

The primary function of this position is to serve as a resident advocate so as to ensure each resident's psycho-social, spiritual needs and wishes are met and maintained from the time of admission through discharge. Primary functions are provided in accordance with federal, state and local regulations and as directed by the Administrator so as to ensure accurate rights and needs of each resident is met on a continuum.

Education and Experience Requirements :

Bachelor's degree in Social Work or Bachelor's degree in a human service field Minimum one year of social work experience in a health care setting. Duties and Responsibilities include but not limited to: Interviews prospective clients and presents all information pertinent to admission to the facility. Makes all necessary arrangements with hospital staff, referring entities, etc., to ensure a timely and smooth admission process. Completes all necessary admission paperwork. Completes all discharge planning making appropriate referrals upon discharge for the residents continuing care, equipment needs, home health, transportation and medications. Participates on the Interdisciplinary Care Committee and completes the initial assessment MDS and care plan for each resident. Evaluates all psychosocial needs of the resident and creates a corresponding plan of care.
