Summary/Objective:
The School Adjustment Counselor is responsible for providing direct therapeutic support services to students and their families. The School Adjustment Counselor will formulate preventative and crisis-oriented treatment plans, participate as an advocate in student related meetings, make referrals for treatment outside of schools, support the families throughout the process and act as a liaison between the teachers, home and school/agencies.
Essential Functions/Duties:
Provides direct therapeutic support services to students and their families. Formulates preventative and crisis-oriented treatment plans including, but not limited to, IEP goals, safety plans, behavior support plans, and behavior contracts. Make referrals for treatment outside of school and assist families with the process. Acts as liaison between school and agency to which referral was made. Participates as an advocate in student related meetings as needed and appropriate. Provides information regarding community resources (including camps) and available services. Coordinates programs to meet educational/social needs of students. Provides liaison between teachers and home. Provides training regarding current laws and regulations related to student safety and transition needs. Assists in the development of curriculum and in-service programs. Makes classroom observations and assists teachers with problem solving techniques. Coordinates other case management as required. Ability to provide case management and clinical support as needed. Other duties as assigned by Program Administrator/Supervisor. •Rate/Salary is determined by educational degree and years of experience.
