This role delivers a variety of employment-related services including Pathways to Work (PTW), Group Supported Employment (GSE), Individual Supported Employment (ISE), and Employment Support Aide (ESA), based on each individual's needs and service authorization. The Job Coach works closely with members, employers, and support teams to promote independence, build job skills, and support successful participation in competitive, integrated employment within the community. Supervisory Responsibilities • None Essential job functions General Responsibilities across all services Provide person-centered support based on each member's goals, preferences, and planning document Promote independence, skill development, and community integration Support members in developing workplace readiness, social skills, and appropriate work behaviors Maintain accurate and timely documentation, service notes, and reporting in compliance with DDD and company standards Communicate effectively with support coordinators, families, employers, and interdisciplinary teams Monitor and report any concerns related to health, safety, or service needs Follow all state, federal, and program compliance requirements Service-Specific Responsibilities Pathways to Work (PTW) Assist members in exploring career interests and identifying employment goals Provide work exploration opportunities and skill-building activities Support members in learning job readiness skills and understanding workplace expectations Help members participate in volunteer or work-based learning experiences Guide members in making informed decisions about employment paths and career options Group Supported Employment (GSE) Support small groups of members in employment or work-related settings Assist members in developing job skills such as punctuality, workplace communication, and task completion Provide supervision and coaching in group work environments Version 0.2 Encourage progression toward competitive integrated employment opportunities Teach workplace behaviors and support interaction with peers and supervisors Individual Supported Employment (ISE) Assist members with job search, application, and interview processes Provide one-on-one job coaching and on-the-job training Support members in learning specific job duties and maintaining employment Gradually reduce support as the member becomes more independent Provide follow-along support to ensure job retention and long-term success Employment Support Aide (ESA) Provide ongoing, on-the-job support for employed members Assist with workplace behavior, task completion, and problem-solving Help members manage personal or social challenges that may impact job performance Communicate with employers and team members to support workplace success Deliver intermittent or as-needed support to maintain employment stability Additional Duties Participate in team meetings, training, and ongoing professional development Support transportation and community access as needed Assist with coordination of services and referrals when appropriate Meet all Arizona Department of Economic Security (DES) / DDD requirements for Direct Support Professionals Complete and maintain all required trainings, certifications (including DCW if applicable), and background clearances Comply with all federal and state regulations (HIPAA, ADA, HCBS) and DDD Qualified Vendor Agreement (QVA) standards Accurately document services, time, and progress in accordance with DDD and billing requirements Follow EVV requirements and participate in audits, monitoring, and quality reviews as required Deliver person-centered services that promote independence, dignity, and community integration Maintain member health, safety, supervision, and timely reporting of incidents or concerns Demonstrate cultural competency and attend ongoing trainings and meetings Knowledge, Skills and Abilities The requirements listed below are representative of the knowledge, skill, and/or abilities required to perform the job: Minimum qualifications High school diploma or equivalent required 1 year of prior experience working with individuals with disabilities or in a related field preferred Prior experience working with individuals with developmental disabilities in a respectful and supportive manner Prior experience teaching, coaching, and modeling appropriate workplace behaviors successfully Prior experience with problem-solving and adaptability in community and work environments Valid driver's license, current vehicle registration and reliable transportation (if applicable) Ability to pass background checks and meet DDD requirements Strong communication and interpersonal skills Version 0.2 Basic computer skills for documentation and communication Working Conditions Community-based settings including workplaces, job sites, and community locations May require standing, walking, and supporting individuals in various environments Flexible schedule based on member needs Travel Required • Travel may be required within the state of Arizona as part of the job duties Physical Requirements Classified as
