04-14-2026 Summary Provides outreach, information-referral, case management, and a full array of mental health /rehabilitation services for clients which include counseling, crisis intervention, skill training, and coordinating and monitoring the assessment, treatment planning process and follow-up for each client. Level II may act as a lead recovery coordinator providing supervision and training to other recovery staff and volunteers.
Essential Duties/Responsibilities Recovery Coordinator I/II:
Conducts intake, completes comprehensive evaluation of each client's life situation, problems and needs, and with client's participation and consent, prepares a detailed plan of care. Responsible for service coordination by assisting clients in implementing their plan of care by facilitating access to and providing community resource linkage and managing the process to include monitoring effectiveness of support services including both client and provider participation. Provides professional support to clients by maintaining regular contact such as through client home visits to provide personal support, consultation, confrontation and intervention, as necessary, including guidance and instruction. Transport clients to accommodate community relationships. Use of personal car or vehicle is required. Monthly Productivity requirement is 100 hours. Maintains daily record of work activities completing daily service progress notes and billing forms per Company and government standards and in compliance with Short Doyle/Medi-Cal requirements related to case management. Conduct outreach, provide information and referral services to persons who do not require or qualify for case management services. Develop and maintain professional working relationships with agencies and service providers. Participates in management, staff and client meetings and training activities as required. Maintain and ensures HIPAA, ethical standards and professional boundaries with all staff and clients. Other duties assigned or necessary to support the program and/or the company.

Provides crisis intervention consultation and other activities directly related to the interventions process Provides individual (adult and children), group and family therapy Provides occupational, education, and other information to enable individual to formulate realistic educational and vocational plans Participates in interdisciplinary treatment planning Participates in program development and staff in-services Participates in staff meetings as required Performs administrative functions consistent with the needs of the program Conducts counseling or therapeutic interviews to assist individual to gain insight into personal problems, define goals, and plan action reflecting interest, abilities, and needs Documents in medical record according to requirements of company and regulatory agencies Evaluates data to identify causes of problem of individuals and to determine advisability of counseling or may refer to other specialist or institutions Maintain and ensures HIPAA, ethical standards and professional boundaries with all staff and clients Follows up results of counseling to determine reliability and validity of treatment used May be required to provide a minimum number of hours weekly, as specified by supervisor or director May teach various psychoeducational classes (parenting, domestic violence, anger management, etc.) and provide on- site therapy in schools Monthly Productivity Requirement is 100 hours or as determined by supervisor/program May be required to spend up to 80% of time counseling individual (adult and children), group and family therapy; students or working with parents/caretakers on and/or off-site Transport clients to accommodate community relationships. Level of Supervision Recovery Coordinator I Given Provides work guidance to case aides. Received Works under immediate supervision of the Recovery Coordinator Supervisor, Clinical Supervisor/Manager, or Program Manager/Director Recovery Coordinator II Directs/supervises the work of case aides and provides performance input. Received Works under immediate supervision of the Recovery Coordinator Supervisor, Clinical Supervisor/Manager, or Program Manager/Director Recovery Coordinator III Given Does not supervise other. Received Works under immediate supervision from Clinical Supervisor and/or Program Director. Receive at least one (1) unit of supervision for every five (5) hours of psychotherapy or counseling performed, per week, in each setting All clinical documentation must be co-signed by a Licensed Practitioner of the Healing Arts (LPHA). Education/Licensing Requirements Education and/or Experience Recovery Coordinator
I A.A./A.S.
degree in human services, psychology or related field and one-year related experience or two years' equivalent experience. Recovery Coordinator
II B.A./B.S.
degree in human services or related field and two years' related experience or four years' equivalent experience. Recovery Coordinator III Current enrollment in a Master's degree program from accredited school in Social Work, Psychology, Counseling, or related field and eligible to complete traineeship hours/courses Approved agreement on file between master's program/school and Pathways Within master's program receiving augmented training and education concurrently to traineeship Currently in good standing with their school and will to participate and/or facilitate communications between agency and school. I/II/III
Proof of:
valid California driver's license and auto insurance, as well as proof of education are required. Prior behavioral health experience preferred. Knowledge, Skills and Experience Recovery Coordinator I Must have minimum one-year equivalent prior case management or psychiatric treatment experience. May be required to have knowledge of rehabilitation model. Intermediate level PC skills required.
Related experience:
coaching, scouts, teacher's aide, or CPS aid. Excellent organizational skills, interpersonal skills, computer skills and communication skills are a must. Ability to effectively prioritize and manage time. Recovery Coordinator II Must have two years' equivalent prior case management experience and prefer established community resource connections. Excellent organizational skills, interpersonal skills, computer skills and communication skills are a must. Intermediate level PC skills required. Ability to effectively prioritize and manage time. Recovery Coordinator III At least two years' equivalent prior case management experience or current enrollment in a traineeship through their Master's program. Excellent organizational skills, interpersonal skills, computer skills and communication skills are a must. Intermediate level PC skills required. Ability to effectively prioritize and manage time. Physical Requirements Office Environment (not remote position). Requires extensive sitting with periodic standing and walking. May be required to lift and/or move up to 20 pounds. Requires significant use of computer, tablet, phone and/or general office equipment. Needs adequate visual acuity, ability to grasp and handle objects. Needs ability to communicate effectively through reading, writing, and speaking in person or on telephone. Will require off-site travel 20% (local and out of state) - meeting, county and/or client needs. Will use personal car or vehicle as required for travel and/ or transportation. Will travel for meetings, training, etc. as needed. However, not remote position.
