Job Description

The Transition Coordinator is responsible for coordinating the options that are available to SNF residents. The Transition Coordinator is responsible for assisting clients in the SNF's who are in need of placement into an ALF or B&C. The transition coordinator will mainly be visiting skilled nursing facilities and hospitals throughout the entirety of the day.
QUALIFICATIONS
Must have a Bachelor's Degree in health care/business administration or related field Previous Health Care experience is preferred Is at least 18 years of age Must have adequate mental and physical health Must have an active CPR License Must have a valid Driver's License Must have the ability to read, write and follow instructions in English Must have the ability to read, write and follow instructions in Spanish Must have good verbal and written communication skills Must have adequate personal computer skills specializing in Excel, Adobe, and PowerPoint Essential Duties and Responsibilities Assist clients in transitioning from the SNF to an ALF or B&C Following up with SNF case managers, discharge planners Establish relationships and provide education to SNF staff members regarding the ALW Program Identifying and interviewing residents for their preferred facilities or locations Reach out to potential ALF or B&C facilities and set up interview between the facility and potential client Create marketing events Maintain proper timekeeping Familiarity with Title 22 Maintain proper credentials Knowledge of mandated reporting Attend all company meetings Will perform other duties as assigned
