May 4, 2026 About the Role Spanish Bilingual Preferred The Recovery Coordinator provides outreach, case management, and mental health rehabilitation services to support Individuals in achieving stability and wellness. This role delivers counseling, crisis intervention, and skill-building services while coordinating assessments, treatment planning, and follow-up care. The position focuses on empowering clients through individualized care and connection to community resources. Responsibilities Conduct client intake and complete comprehensive assessments of client needs and life situations Develop individualized treatment plans in collaboration with clients and families Coordinate and manage services to ensure effective implementation of care plans Provide counseling, crisis intervention, and skill-building services Maintain regular client contact, including home visits, to provide support and intervention Facilitate access to community resources and monitor effectiveness of services Transport clients as needed to support community integration (use of personal vehicle required) Maintain daily documentation including service notes and billing in compliance with regulatory standards (Short Doyle/Medi-Cal) Conduct outreach and provide information and referrals for individuals not enrolled in case management Maintain compliance with HIPAA, ethical standards, and professional boundaries Provide temporary support to former clients as needed Build and maintain relationships with community agencies and service providers Participate in staff meetings, trainings, and program development activities Meet monthly productivity requirement of 100 hours Perform additional duties as assigned Recovery Requirements Provide the highest level of customer care Identify and build on strengths of clients, staff, and communities Support client recovery and wellness journeys Promote a culture that values diversity and inclusion Embrace continuous learning and professional development Utilize evidence-based practices to ensure optimal outcomes Required Qualifications AA, AS, or BA degree in human services, psychology, or related field and one year of related experience, or two years of equivalent experience Minimum one year of case management or psychiatric treatment experience Valid California driver's license and active auto insurance Knowledge of rehabilitation model preferred Strong organizational and prioritization skills Excellent interpersonal, written, and verbal communication skills Intermediate computer proficiency Related experience such as coaching, scouting, teacher's aide, or CPS aide is a plus
