Position Overview We are seeking a passionate and dedicated Case Manager to join our team. In this role, you will provide direct support and case management services to individuals with developmental disabilities. The ideal candidate will have a strong commitment to person-centered care, advocacy, and the ability to foster positive relationships with clients and their families. We are currently hiring Case Managers in both South Bay and North County areas, including but not limited to Chula Vista and Vista, CA. Candidates who can serve these regions are highly encouraged to apply! Responsibilities Conduct comprehensive assessments to identify individual needs, strengths, and goals. Develop and implement person-centered service plans in collaboration with clients, their families, and interdisciplinary teams. Provide direct support to individuals with developmental disabilities, including assistance with daily living activities, skill development, and community integration. Advocate for clients' rights and access to community resources, ensuring their voices are heard and respected. Monitor and evaluate clients' progress, adjusting service plans as needed to achieve desired outcomes. Collaborate with community partners, service providers, and healthcare professionals to enhance the overall support network for clients. Maintain accurate and up-to-date documentation, including progress notes, reports, and case records. Participate in team meetings, training sessions, and professional development opportunities to stay informed about best practices and industry trends. Qualifications (Bilingual applicants highly encouraged to apply) High School Diploma or equivalent (training is provided upon hire). Experience working with individuals with developmental disabilities, preferably in a case management or direct support role is a plus but not required; training is provided upon hire with a dedicated leadership team. Strong understanding of person-centered planning and advocacy principles. Excellent communication and interpersonal skills. Ability to work independently and as part of a collaborative team. Knowledge of local community resources and services for individuals with developmental disabilities. Compassionate, patient, and culturally sensitive approach to client care. Reliable transportation to commute to client homes and/or job sites. Compensation/Benefits $18.50/hour mileage reimbursement Paid Sick Leave after 90 daysState-Sponsored Retirement Plan Job Types:
