Duties and responsibilities Provides direct, hands-on assistance to identified clients in the accomplishment of activities of daily living, including meal planning, grocery shopping, food safety, cooking simple nutritious meals, laundry, cleaning, and maintaining a safe and sanitary living environment. Demonstrates, models, and coaches household routines such as cleaning schedules, organization, personal care, and basic budgeting related to food and household supplies . Supports clients in achieving personal treatment goals and service plan objectives through skill-building, practice, encouragement, and reinforcement. Assists clients in developing independent living skills, including tenant responsibilities, understanding house rules, and maintaining housing stability. Provides transportation for identified clients to medical, behavioral health, employment, grocery shopping, food pantries, and other essential appointments in accordance with agency policy. Develop weekly schedule of activities for clients based on identified needs to promote structure and consistency. Engages clients in community integration activities, including libraries, community events, appointments, and other social or educational opportunities. Facilitates individual and group life skills activities addressing topics such as hygiene, food literacy, financial basics, employment readiness, wellness, tenant rights, and relationship building. Coordinates and advocates for the involvement of appropriate professionals and community resources. Communicates with collaborative agencies regarding changes in client mental, physical, or housing status. Conducts outreach and engagement with individuals experiencing homelessness when assigned. Schedules and conducts house meetings as required. Assists with unit transitions, move-ins, and clearing or organizing resident belongings when a resident vacates. Maintain safety and security by monitoring the environment of care and responding appropriately to emergencies in accordance with agency policies and procedures. Completes all assigned documentation accurately and timely within 48 hours of service delivery in the Electronic Medical Record and other required systems. - Check company email at least once per day, responding timely, appropriately, and professionally to communicate internally and externally. Maintains professional boundaries, confidentiality, and ethical conduct at all times. Performs other duties as assigned in support of agency goals and program outcomes. Qualifications High School Diploma or GED required. One to three years experience providing direct services to adults with serious mental illness, substance use disorders, and/or histories of homelessness preferred. Ability to provide hands-on coaching in activities of daily living and independent living skills. Must have knowledge of community resources serving individuals with behavioral health and housing needs. Demonstrates proficiency in verbal and written communication. Must demonstrate proficient computer skills (40 wpm). Must be able to effectively utilize electronic health records. Must demonstrate adequate knowledge and use of Microsoft Outlook, Word, and Excel. Ability to work effectively with diverse populations and with minimal supervision . Must maintain all required trainings. Must successfully complete all criminal background screens and remain eligible.
