Non-Exempt Position Summary The Supported Living Coach provides individualized support and training to persons with intellectual and developmental disabilities who live independently in their own homes or apartments. The Supported Living Coach assists individuals in developing and maintaining the skills necessary to achieve maximum independence, self-determination, community inclusion, health, safety, and quality of life. Essential Duties and Responsibilities Provide individualized training and support based on each person's support plan and personal goals. Assist individuals with budgeting, money management, and banking activities. Support meal planning, grocery shopping, and food preparation. Assist with household management, including cleaning, laundry, and home organization. Teach and reinforce personal hygiene, health, and wellness practices. Support individuals in scheduling and attending medical, dental, and other appointments. Assist individuals in accessing community resources and participating in community activities. Provide transportation or travel training as authorized. Support individuals in obtaining and maintaining employment or volunteer opportunities. Assist with housing-related responsibilities, including communication with landlords and utility providers. Monitor health and safety concerns and report incidents according to agency and APD requirements. Maintain accurate documentation, progress notes, and service records. Advocate for individual rights, choices, and personal goals. Promote independence while providing only the level of assistance necessary. Participate in staff meetings, training, and required professional development activities. Maintain confidentiality in accordance with HIPAA and agency policies. Qualifications Education High School Diploma or GED required. Associate degree in Human Services, Social Work, Psychology, or a related field preferred. Experience Experience working with individuals with intellectual or developmental disabilities preferred. Experience in community-based services, residential services, or case management is beneficial. Certifications and Requirements Valid Florida Driver's License with an acceptable driving record. Current CPR, First Aid, and required APD certifications. Successful completion of Level II Background Screening. Ability to meet all agency and state training requirements. Knowledge, Skills, and Abilities Ability to teach independent living skills. Strong communication and interpersonal skills. Ability to work independently with minimal supervision. Knowledge of person-centered practices. Ability to maintain professional boundaries. Basic computer and documentation skills. Ability to problem-solve and make sound decisions. Commitment to promoting dignity, respect, and individual choice. Physical Requirements Ability to stand, walk, bend, and lift up to 25 pounds. Ability to assist individuals with community activities and transportation. Ability to travel throughout the service area as needed. Work Environment Work is performed primarily in individuals' homes and community settings. The position requires flexibility in scheduling, including evenings, weekends, and holidays as needed to meet the needs of the individuals served. Performance Expectations Maintain compliance with APD regulations, Medicaid Waiver requirements, and agency policies. Complete documentation accurately and within required timelines. Demonstrate professionalism and positive interactions with individuals, families, coworkers, and community partners. Promote individual independence and community inclusion at all times. Maintain satisfactory attendance and reliability.
