Full Job Description Description:
The Lead Outreach Intervention Specialist will lead a team conducting outreach both on foot and by van. The team will identify target populations for engagement in project services; conduct assessments, create service plans, engage and link clients with apprpriate services in hopes of connecting homeless individuals to need treatment and housing resources. The Lead Outreach Specialist is also responsible for establishing relationships with key stake holders and making program decisions that will assist in meeting program goals.
ESSENTIAL FUNCTIONS TO INCLUDE
; Provide direct outreach alongside outreach team to clients by providing food, clothing, transportation, case management and referrals for housing and shelters among other needed services. Provide trainig and guidance to all team members on conducting brief screenings and identyfing clients with substance use and/or mental healthdisorders to link them with appropriate resources. Participate in the annual homeless point in time count. Participate in Coordinated Entry System activities. Completion of documentation in appropriate database (HMIS, EHR) in a timely manner to ensure thorough and complete reporting to funders. Conduct outreach with provider organizations who may wish to refer clients to treatment. Follow agency regulations regarding van use. Participate in ongoing staff development traiining. Attend all staff meeting and scheduled supervisions. Follow all safety protocols. Other duties as assigned.
Requirements:
Education, Experience and Skills required: Previous Experience working in homeless, substance use, and mental health population required. HS Diploma or GED equivalent required. BS Social Work or Health related field preferred. Valid Illinois drivers license and ability to operate a van. Must have previous experience in outreach and/or case management services
