Work with employers to solve conflicts, establish natural job supports, and train them how to communicate effectively with persons with developmental disabilities. Keep accurate and timely documentation of sites worked according to specific documentation requirements. Use own vehicle to transport persons to/from sites as needed. Will follow Rule 50, Dept. of Human Services Rule 115, Rule 116, Rule 119, Rule 120, and other relevant regulatory guidelines. Essential Duties / Responsibilities Mission, Vision, & Values To support the philosophy through the agency's mission, vision, values, and strategic plan. Manage all responsibilities based on the agency mission, using individual choice and all appropriate policies, procedures, and regulations. To conduct oneself in a manner that reflects the highest standards of professional courtesy and ethical practices to people served, staff, guardians, families, and all outside contacts. Program/Department development and Coordination Provide program instruction to assist in attaining and maintaining a level of self-sufficiency necessary to participate in community employment. Provide direct training and instruction to individuals to meet community employer's standards and job specifications. Monitor client work schedules to ensure proper attendance and punctuality. Provide transportation for the clients as needed. During the transitional phase from a sheltered to a community environment, facilitate and encourage active integration with co-workers and supervisors. Maintain regular contact with community employers to enhance communication and develop natural supports. In cooperation with supervisor develop and follow the individualized written task analysis for each community job site. At least monthly, consult with identified IDT members for clients in the community to discuss progress, strengths, needs, and objectives. As requested, and scheduled, participate in skills training and upgrading of technical knowledge through Agency in-service training, local workshops, and community seminars. Attend staffing and departmental meetings as requested. Education/Experience Requirements Must be able to communicate effectively both verbally and in writing. Must be able to assess physical environment based on health and safety needs and implement corrective measures when needed. Knowledge of the principles of Person-Centered Planning and the Council of Quality Leadership (CQL). Competency Requirements Ability to manage others. Ability to multi-task.
