Job Description

Are you passionate about helping individuals through some of the most challenging moments of their lives? Join our Stabilization Team and become a vital source of support for people experiencing mental health and addiction crises. As a Crisis Case Coordinator, you'll play a critical role in providing immediate assistance, crisis intervention, and ongoing support to individuals in need. This rewarding position combines compassionate care, problem-solving, and case management to help clients connect with the resources and services they need to achieve stability and recovery. To support a safe and healthy environment for the individuals we serve and for one another, all employees of Radiant Health are required to comply with organizational health and infection prevention requirements as a condition of employment. This includes providing documentation of immunization status, completing required health screenings, and adhering to communicable disease prevention protocols applicable to the role and work environment. Radiant Health will review requests for reasonable accommodation in accordance with applicable federal and state law. Bachelor's degree in social work, psychology, counseling, or a related field. Minimum of 2 years of experience in crisis intervention, case management, or a related field. Experience working in rapid response or emergency services preferred. Relevant licensure or certification preferred. Valid driver's license and reliable transportation for community-based work. Ability to work flexible hours, including evenings, weekends, and on-call shifts as needed.
