Job Description

NFI Massachusetts, Inc., a nonprofit human services organization, is seeking an engaging and motivated Transitional Specialist to support justice-involved individuals as they transition back into the community through the Supporting Transitions & Reentry (STAR) program, located in Lynn and Lawrence, MA, under the jurisdiction of the Essex County Sheriff's Department. This role focuses on active outreach, engagement, and skill-building to help individuals achieve stability, independence, and long-term success. The Transitional Specialist plays a critical role in connecting individuals to essential resources, reducing barriers to care, and supporting overall wellness and community integration.
Key Responsibilities:
Conduct outreach and engagement with assigned individuals in the community Build and maintain strong connections with local resources and referral networks Assist individuals in overcoming barriers to services (e.g., transportation, language, financial challenges) Support applications for employment, housing, and public benefits Accompany individuals to court appearances and other appointments as needed Provide coaching, guidance, and accountability to support program participation and goals Collaborate on individualized service and treatment planning Participate in group facilitation and team meetings as appropriate Coordinate transitions between services and providers Maintain accurate and timely documentation in the electronic health record (EHR) Ensure compliance with all NFI Massachusetts and ECSD policies and procedures Organize volunteer activities in the community Research and connect participants with free or reduced-cost recreational activities
Qualifications:
Bachelor's degree preferred (or equivalent relevant/lived experience) Experience in workforce development, reentry services, or related human services field Strong engagement and relationship-building skills Ability to work independently in community-based settings Strong organizational and communication skills
Pay Range:
