Job Description:

Develop job site opportunities for individual employment. Support adults with disabilities or other barriers to gain competitive employment in the community. Develop career goals, prepare for their job search, secure their job, and coach the individuals as they learn the tasks and responsibilities. On-site support with community activities, including Vocational Assessment, Job Placement, and Job Coaching in paid community employment. Develop and maintain relationships with local businesses and stakeholders. Write daily notes & track monthly progress/goals. Case management. Track support hours.

Minimum Requirements:

Candidates who are passionate about working with individuals with disabilities or desire to work with a human services organization. A successful candidate will have a personal vehicle, driver's license, and proof of being listed as a covered driver under an auto insurance policy. Candidates must have a clean criminal and driving record in to attain employment with Transitions Inc. A Bachelor's Degree is preferred for this position but is not mandatory. In addition, all candidates must possess an acceptable CORI and SORI check, DPPC Screen, fingerprinting screen & motor vehicle safe driving check will be required for candidates seeking employment.
