Job Description
General Description of Duties:
The Job Coach is responsible for providing training and assistance to individuals by providing opportunities to engage and secure employment commensurate with the Individual's skills and experience. In addition, the Job Coach will continually explore alternative work options through job development efforts. The Job Coach will also complete relevant documentation as well as any other directives specified in aPerson-Centered Plan Essential Tasks and Responsibilities:
In a manner, consistent with supporting Langton Green, Inc. Mission Statement, the Job Coach shall do the following: Ensure all individuals served are living according to their rights and responsibilities and are free from abuse and neglect. Communicate and interact positively and effectively with each person receiving supports, visitors, family members, peers, and supervisors. Assist, support and coordinate Individual's medical appointments as necessary with job responsibilities Administer Medication as needed Provide formal coaching for job to include training an individual how to complete jobs, or component tasks, or observe as the supervisor trains the individual(s). Provide job development for new admissions or if individual[s] in the program are looking for alternative employment or volunteer efforts. Be available to work flexible shifts on a short-term basis to train individuals in new evening or weekend positions and or to respond and assist with new job duties or issues that develop in an existing position. Provide appropriate feedback and instructions on how the individual[s] should interact with co-workers and supervisors Observe the job culture of the environment so that you can help the individual[s] to fit in with co-workers Assist and support the individual[s] to develop his own system of organizing and remembering the job so that you can fade (reduce support as necessary) Mediate differences with co-workers or employer if necessary, and assist individual[s] in developing the skills and coping mechanisms to resolve his/her own difficulties Meet with individual[s] prior to work, breaks or right after work to eliminate any on-the-job distractions Assist and support individual[s] to develop plans for managing, saving and spending the money that they earned Complete program notes, daily attendance, Incident Reports, and document goals and outcomes for the individual[s] Individual Plan and utilize all other available documentation options that best serve the individual[s] Assist in completion of jobs/tasks related to ongoing or contracted vocational activities to ensure timely completion. Ensures all deadlines are met. Respond to all phone calls and emails within 24 hours. Other duties as assigned.Qualifications:
Knowledge and understanding of the needs of individuals with disabilities is preferred. Must have demonstrated abilities in supervision, budget management, and service delivery. Must be able to demonstrate skills in the following areas: problem solving, project/task management, time management, dependability, good judgment and safe work practices. Current maintenance of all required training's under DDA and CMT guidelines and Langton Green's. Flexibility of scheduling to meet Agency and program needs. Education and/orExperience:
AA Degree in or High School Diploma (or equivalent). Two years' experience in the human service field. Two to Five years in Residential or Personal Support Services environment and Two to Five years of experience in a supervisory role. Fluent in Microsoft Office Suite (Excel, PowerPoint, Outlook and Word).Language Skills:
Ability to read and interpret documents. Ability to write reports and complete forms or other documents as required. Ability to communicate with others including individuals in Langton Green's programs, family members, medical professionals, stakeholders and co-workers.Certificates, Licenses, Registrations:
Must have valid Motor Vehicle Class "C" Driver's License and access to reliable transportation. Maintenance of all required training's under DDA and CMT guidelines and Langton Green's.Physical Demands:
While performing the tasks of this position, the employee is required to stand, walk, sit, bend, kneel, and use hands to finger, handle or feel, reach with arms and hands, balance, lift, stretch. Must be capable of lifting/moving over 50 pounds and the transferring of individuals with or without equipment. Vision abilities required for this position include, near distance, far distance, peripheral vision, color vision, depth perception and the ability to adjust focus.Other:
