This role ensures access to community resources, entitlements, and supports that promote independence, self-determination, and quality of life. Responsibilities include developing and implementing Individual Plans of Service (IPOS), coordinating care, and fostering community connections. Key Responsibilities Facilitate and implement Person-Centered Plans Link, coordinate, and monitor services outlined in the IPOS Assist with maintaining entitlements (Medicaid, Medicare, SSI, etc.) Support crisis planning and resolution Coordinate health and safety strategies, including discharge planning Promote community inclusion, self-advocacy, and relationship building Assist with housing, transportation, and employment resources Collaborate with schools for transition planning Complete timely documentation, assessments, and progress notes Participate in required trainings and professional development Qualifications Bachelor's degree in Human Services or related field Minimum one year of experience supporting individuals with I/DD Knowledge of Person-Centered Planning and Self-Determination principles Strong communication, organizational, and computer skills Valid Michigan driver's license, insurance, and acceptable driving record Additional Requirements Extensive travel within Wayne County; reliable transportation required Ability to work flexible hours, including adjusted schedules as needed Completion of all required trainings, including QIDP-credited training annually About Community Living Services (CLS) CLS provides person-centered supports for individuals with intellectual and developmental disabilities in Wayne County.
