Job Description

Position Overview Seeking a qualified and experienced Designated Coordinator (DC) and/or Designated Manager (DM) to oversee and support Home and Community-Based Services (HCBS) under Minnesota 245D licensed services. The ideal candidate will have strong knowledge of Minnesota
DHS 245D
regulations, person-centered service planning, compliance requirements, staff supervision, and waiver service coordination. This role is responsible for ensuring services are delivered in compliance with Minnesota Statutes Chapter 245D and DHS standards while supporting individuals receiving HCBS services in community settings. Job ResponsibilitiesDesignated Coordinator Duties Coordinate and monitor HCBS services for individuals receiving support Develop, review, and maintain: CSSPs CSSP Addendums Support plans Progress reviews Person-centered documentation Conduct intake meetings and annual service planning meetings Ensure documentation complies with Minnesota 245D and DHS standards Monitor implementation of individualized goals and outcomes Train and support direct support staff Participate in incident reporting and follow-up procedures Maintain communication with: Case managers Guardians/families Residential providers County representatives Ensure health, safety, and rights protections are maintained Complete required service supervision and quality assurance activities Designated Manager Duties Supervise and support Designated Coordinators and Direct Support Professionals Ensure agency-wide compliance with: Minnesota Statutes Chapter 245D HCBS Waiver requirements DHS licensing standards Oversee staff training, evaluations, and competency requirements Maintain policies, procedures, and quality improvement systems Assist during DHS licensing reviews, audits, and site visits Ensure incident management and corrective action processes are followed Support operational compliance and program oversight Minimum Qualifications Required by Minnesota

Statute Designated Coordinator Qualifications Per Minnesota Statutes 245D requirements, qualified applicants must meet ONE of the following: Option 1 Bachelor's degree in: Human Services Psychology Sociology Social Work Education Nursing Or related human services field Option 2 Associate degree in a related field AND Minimum of 2 years of full-time work experience providing direct care services to persons with disabilities or related populations Option 3 Diploma in a related field from an accredited post secondary institution AND Minimum of 3 years of full-time experience providing direct support services Option 4 Minimum of 50 hours of education and training related to human services and disabilities AND Minimum of 4 years of full-time experience providing direct care services to individuals with disabilities or related conditions Designated Manager Qualifications Applicants for Designated Manager must meet ONE of the following: Option 1 Bachelor's degree in a related field AND Minimum of 3 years supervisory experience in human services or disability services Option 2 Associate degree in a related field AND Minimum of 4 years of experience in human services, including supervisory responsibilities Option 3 Diploma in a related field AND Minimum of 5 years of experience in disability or human services with leadership responsibilities Option 4 Minimum of 6 years of combined education and experience in human services with demonstrated competency in program oversight and staff supervision
