Job Description

Oakridge Homes promotes dignity and independent living for people with developmental disabilities and mental health needs. EOE Oakridge Homes is currently looking for a Designated Coordinator who :
  • Meets the qualifications of a DC in education and work experience by providing direct care services to persons with disabilities or persons aged 65 or older as follows: a.
a baccalaureate degree in a human services related field, and 1 year of full-time work. b. an associate degree in a human services related field, and 2 years of full-time work. c. a diploma in a human services related field from an accredited postsecondary institution and 3 years of full time work; or d. a minimum of 50 hours of education and training related to human services and disabilities; and 4 years of full time work under the supervision of a staff person who meets the qualifications identified in any of the first three described above.
  • Has excellent verbal/written communication skills
  • Will office based out of Clarissa, MN with work and day travel primarily in Todd and Wadena counties and the surrounding area
  • Can provide a valid MN driver's license and clean driving record
  • Can pass a DHS background study As a Designated Coordinator, you will:
  • Perform highly responsible duties related to the development, implementation and coordination of Individual Service Plans for persons served with developmental disabilities or mental health needs
  • Maintain, review and evaluate pertinent program data related to each individual's growth and development
  • Ensure that services are being correctly implemented
  • Provide training to direct care staff as indicated Oakridge Homes offers:
