Housing Supplemental support specialist Company Overview At Residential Transitions, Inc. Through compassionate, person-centered care, we aim to support long-term stability and meaningful progress in the lives of those we serve. Position Overview Residential Transitions, Inc. (RTI) is hiring a Part-Time Housing Support Supplemental Services Specialist to provide on-site support services in a licensed residential setting under Minnesota Housing Support (MN Statute 256I) and Hennepin County Housing Support requirements. This role focuses on helping individuals maintain stable housing through daily living support, tenancy skill development, and person-centered case coordination. Services are provided in alignment with the client's Professional Statement of Need (PSN) and must be documented through compliant case notes as required by DHS and county agreements. Role Responsibilities Provide Housing Support Supplemental Services to individuals in a residential setting Support clients with tenancy-related skills including lease compliance, landlord communication, and understanding tenant responsibilities Assist clients with daily living skills such as household management, budgeting, organization, and problem-solving Provide oversight and support with basic health and safety needs as outlined in the PSN Assist with scheduling and coordination of appointments, transportation, and community resources Monitor client well-being and report changes in behavior, housing stability, or risk factors Use person-centered, trauma-informed, and harm reduction approaches in all interactions Support clients in developing independence and maintaining stable housing Maintain timely, accurate, and compliant case notes for all services provided Ensure documentation meets Minnesota DHS and Hennepin County Housing Support requirements Collaborate with case managers, county workers, and other providers to support client goals Participate in team meetings, case reviews, and program quality activities Perform other duties as assigned Required Qualifications and Experience Experience working with individuals with mental illness, substance use disorders, housing instability, or complex behavioral health needs Strong communication, organization, documentation, and time management skills Ability to pass DHS background studies and required criminal background checks Ability to work in a residential, on-site setting with direct client contact Understanding of person-centered and trauma-informed care practices Ability to maintain accurate and timely case documentation in compliance with county standards Ability to work independently while maintaining accountability and service quality Ability to work effectively with culturally diverse populations Valid driver's license and reliable transportation (if transportation duties are required) Preferred Qualifications Experience in Housing Support, 245D settings, case management, or residential support services Knowledge of Minnesota DHS Housing Support Supplemental Services requirements Experience supporting individuals with tenancy stabilization needs Training in crisis prevention, de-escalation, or behavioral support strategies Physical & Mental Requirements Employees in this role must be able to: Work in a residential, community-based environment Sit, stand, walk, bend, and climb stairs throughout the shift Use computers and electronic documentation systems for case notes and reporting Communicate effectively with clients, staff, and external partners Lift up to 25-40 lbs as needed Respond appropriately in behavioral or crisis situations when required Maintain awareness of client safety and environmental conditions
