Support daily operations of Day Habilitation Services and assist in delivering person-centered programming. Assist in supervising, training, scheduling, and supporting Day Habilitation staff. Help develop, implement, and monitor individualized Day Hab plans and outcomes. Coordinate community volunteer opportunities and meaningful integrated activities for individuals supported. Collaborate with families, interdisciplinary teams, and community providers to ensure coordinated services. Monitor service delivery at home base and community sites and coordinate transportation supports. Complete and maintain required documentation, reports, and compliance records. Promote a professional, inclusive, and person-centered environment for staff and individuals supported. Assist in obtaining equipment and resources necessary to support effective services.What You Bring:
Bachelor's degree in a related field with one (1) year of experience supporting individuals with disabilities; OR Associate degree with three (3) years of related experience; OR High school diploma/equivalent with five (5) years of related experience, including supervisory experience. Strong communication, leadership, and organizational skills. Ability to interpret policies, service plans, and regulatory requirements. Demonstrated problem-solving and decision-making abilities. Must meet Lifetime Assistance Vehicle Operator Requirements. Ability to meet physical requirements of the role, including lifting up to 50 pounds and maintaining SCIP-R certification if required.Our Mission & Culture:
