Job Description
Responsible for effective case management in an outpatient substance use program. Will assist clients in accessing and resolving barriers to resources, referrals to appropriate providers, coordinating care with outside entities, and benefitting recovery activities. Will also assist with transition program.

What Youll Do:
Coordinate and deliver high-quality, timely, and equitable care in collaboration with offsite supportMaintain a caseload and stay connected with clients through ongoing contact and progress monitoringCreate individualized service plans and support clients through advocacy, crisis intervention, and referralsProvide assistance with housing, budgeting, transportation, medication monitoring, and moreFacilitate group sessions or educational workshops as neededDocument clinical activity clearly and on time to meet all compliance standardsParticipate in treatment planning and contribute to decision-making on client careHelp individuals move toward independence through supportive counseling and resource linkageWhat You Bring:
A Bachelors Degree in Behavioral Science or related field, or equivalent experience (as permitted by regulations)Valid drivers license and auto insuranceStrong interpersonal and communication skillsyou connect with people from all walks of lifeThe ability to stay organized, manage multiple responsibilities, and solve problems with confidenceComfort using technology and electronic documentation systems (Microsoft Office proficiency preferred)Essential Functions/Physical Demands:
