Summary Works in the community with patients suffering from substance use on a peer-to-peer basis. Helps gain access to needed resources in the community by assisting in overcoming barriers and helping bridge gaps between needs and available resources. Job Duties Develops supportive peer relationships with people with substance use disorder, serving as a role model, mentor, and motivator to offer insight into the recovery process, promote safety, and reduce harm due to addiction. Promotes relationship-centered, trauma-informed services and recovery-orientated environment; assists individual and their family in making decisions, taking action, and treatment planning Communicates with the patient and/or patient's family to enhance the understanding of addiction as a chronic disease and available treatment options. Employs CRS skills to provide tailored support services options for patients given their individual circumstances and readiness for change. Assists with the development of a recovery management plan; identifies patient strengths and community resources to overcome barriers and achieve recovery goals, determining progress at designated intervals. Works as a member of an interdisciplinary team to support comprehensive treatment plans with relevant measurable problems, goals, objectives, interventions, and timelines. Initiates referrals for services and helps patients with substance use by assisting with access to community resources. Leads in recovery activities such as mentoring, securing a sponsor, and engaging in one-on-one and group recovery sessions. Maintain project logs, reports, and accurate records in appropriate files and database(s), adhering to program standards. Facilitates transportation for patients to appointments or treatment locations. Participates, in conjunction with community services and advocacy programs, in educating and fostering awareness in the community about the disease of addiction and substance use disorder. Minimum Qualifications High School Diploma/GED 1 year lived experience as a consumer of SUD services. Ability to work flexible schedules outside of normal business hours to meet the needs of clients. Knowledge of community agencies and support services. Proficient in Microsoft Office applications. Sound problem solving skills and proactive efforts in seeking solutions to problems. CRS
