- DuBois Area PA Connecting Communities (PACC) is seeking a dedicated, enthusiastic, and compassionate Habilitation Specialist to provide one-on-one support to individuals with intellectual disabilities in home and community settings.

Our vision is to create social change through vocational (on-the-job) training programs, social and cultural activities, recreational and fitness opportunities, and remedial/academic classes for the disability community. PACC follows the principle of "Everyday Lives." The Habilitation Specialist plays a vital role in supporting this mission by assisting participants in learning skills, engaging in their communities, and achieving the goals outlined in their Individual Support Plans (ISP). This is a highly rewarding position for individuals who are passionate about person-centered support, community inclusion, and helping individuals reach their fullest potential. Duties include, but are not limited to: Provide direct support to individuals with developmental disabilities, assist with activities of daily living (ADLs), and promote independence. Follow all ODP and PACC policies, procedures, confidentiality requirements, and professional standards. Implement ISP goals and document daily outcomes and same‑day progress notes. Lead participant‑directed and community activities, teach assigned skills, and safely transport clients. Communicate effectively with clients, families, and supervisors, and provide weekly participant updates. Monitor health and safety, report incidents, and assist with mobility needs. Submit weekly timecards, expenses, receipts, and required documentation. Coordinate with the supervisor for planning, schedules, and support‑coordinator contact approval, and maintain an up‑to‑date planning calendar.
Applicants must:
Be at least 18 years old Have a minimum of a high school diploma or GED equivalent Have experience in adult education, mental health, psychology, or ID services (preferred but not required) Maintain a valid driver's license, reliable vehicle, insurance, and registration Be physically able to assist participants with mobility needs Be smart phone and computer literate Be able to pass Criminal Background Check, Child Abuse Clearance, and FBI check
