Job Description

Keystone Mental Health Services is currently seeking a full time Clinic Specialist (Skill Builder Specialist), to join our team in providing individualized, person-directed support for our Transitional Age program. Our work focuses on inclusion and building communities where everyone has the opportunity to thrive and fully participate in everything society has to offer. The Clinical Specialist is a clinically focused role responsible for teaching essential skills to young adults with Mental Health using Positive Behavior techniques, systematic instruction, and ongoing assessment. This position develops and implements individualized strategies that promote growth, independence, and meaningful progress. The Clinical Specialist also provides consultative guidance and mentorship to team members, supporting high‑quality service delivery and consistent use of evidence‑based practices across the program.
Responsibilities Include:
Develops, updates, and maintains comprehensive assessments for each individual to ensure accurate evaluation of skills and needs Collaborates with team members to create individualized annual plans and monitors plan implementation Provides training, guidance, and support to employees delivering habilitation services Delivers face‑to‑face, and administrative skill‑building services for adults with Mental Health Creates Skill Building Plans (SBPs) to support independence in education, employment, and self‑advocacy Conducts baseline skill assessments and analyzes results to determine targeted outcomes Reviews data regularly to update plans and assessments based on progress and emerging needs Identifies and coordinates additional services to support individual needs Meets with individuals, families, and team members to train on SBPs, review progress, and address concerns Participates in support service meetings and contributes to annual service planning Communicates consistently and professionally with individuals, families, team members, and health professionals Recommends updated materials, environmental adjustments, or new strategies to enhance progress and maintain outcomes Maintains accurate records and required documentation for all assigned individuals Provides transportation for individuals supported when needed Supports evaluation and mediation between outside employers and individuals after job placement to address work‑related concerns
Minimum Requirements:
One of the following: Bachelor's degree in psychology, Human Services, Social Work, Education or related Human Services field Bachelor's degree in an unrelated field with three years experience in human services field Effective communication and proficient computer skills Valid driver's license with daily access to a privately maintained and insured vehicle Successful completion of the pre-employment process, including clearances and verifications
Job Details:
Full time, non-exempt position $23.50 per hour Keystone Human Services values health, wellbeing, and professional growth.
