HHCI is in search of a Vocational Specialist (VS) for its Assertive Community Treatment (ACT) Team. This position is field-based, and will require the VS to report to the office (located in Baltimore, MD) as needed. The need to report to the office will be determined and communicated to the VS by the Clinical Director (CD)/CEO.
Position Summary :
The Assertive Community Treatment (ACT) team VS provides ACT services to a mutually shared caseload of ACT clients within HHCI's treatment and recovery services division. The VS is responsible for utilizing evidence-based practices to assist their clients with meeting their treatment and recovery goals, and operating within the scope of their profession. This includes providing individual placement and support (IPS) services, psychoeducation, motivational interviewing, psychosocial rehabilitation support, and trauma-informed care.
Responsibilities :
Provide ACT services to each admitted client of the ACT team at least monthly Conduct vocational assessments within 30 days of admission for newly admitted clients, and every six months thereafter for existing clients Conduct psychosocial assessments within 30 days of admission for newly admitted clients, and annually thereafter for existing clients Conduct additional mental health assessments as needed and directed by the CD Coordinate referrals for additional support for admitted clients Accompany clients to appointments, if needed Enter, sign, and submit all electronic clinical documentation to the CD within 24 hours of the encounter with the client, including progress notes and assessments Conduct intake screenings for prospective clients Prepare for, attend, and participate in daily team meetings Engage in person-centered treatment planning with clients Conduct requested self-audits of charts for assigned clients, as well as unplanned chart audits Collaborate with consented external providers and internal personnel regarding the treatment goals of mutually shared clients Complete required trainings within established timeframes Rotate the 24-hour on-call ACT phone with other members of the team Comply with all relevant standards and regulations of HHCI, the state of Maryland's Behavioral Health Administration (BHA), The Joint Commission, HIPAA Perform other duties as requested by the
CD Required Qualifications :
Bachelor's Degree in Social Work (BSW), Psychology, Human Services, or a related field, AND A minimum of 1 year of providing educational and employment support to the mental/behavioral health population, and functioning in the role of a vocational/employment counselor OR Associate's Degree in Human Services, or a related field, AND A minimum of 3 years of providing educational and employment support to the mental/behavioral health population, and functioning in the role of a vocational/employment counselor Proficiency in Microsoft Suite (Word, Excel, Outlook, Power Point, Access), electronic health records, and the use of computers/laptops
Preferred Qualifications :
Strong preference for a Master's Degree in Social Work (MSW), Mental Health Counseling (MHC), Marriage and Family Therapy (MFT), Creative Arts Therapy (CAT), Counseling Psychology, Clinical Professional Counseling, or a related field Strong preference for experience working with a multi-disciplinary mobile team
