Bronx, Brooklyn, Manhattan, Queens, and Staten Island Join Our Team at A1 Service Coordination A1 Service Coordination is a conflict-free agency supporting individuals in the Traumatic Brain Injury (TBI) and Nursing Home Transition & Diversion (NHTD) Medicaid Waiver programs. We serve participants across New York City and the Lower Hudson Valley. We're hiring a full-time Service Coordinator (SC) based in New York City to provide person-centered, community-based support to individuals with disabilities, TBI, and older adults. Important Travel Requirement This role requires regular in-person home visits across all five boroughs: Bronx, Brooklyn, Manhattan, Queens, and Staten Island. Candidates must be willing and able to travel to all boroughs as needed. This is a hybrid position combining remote work with field visits. Candidates should be comfortable using public transportation or other reliable means to travel throughout NYC and entering participants' homes. Who We're Looking For Experienced professionals who are comfortable working independently in the field and are passionate about supporting individuals in maintaining independence in the community. Spanish, Russian, or other language skills are strongly preferred. Why Work With Us Hybrid remote role with flexible scheduling Paid training and professional development PTO and paid holidays Transportation reimbursement Health stipend and 401(k) Laptop and technology provided Supportive, collaborative team environment Opportunity to make a direct impact Position Overview Manage a caseload of up to ~25 participants in the TBI and NHTD Waiver programs Conduct in-person home visits and monthly contacts Develop and maintain individualized service plans in compliance with waiver requirements Assist participants in accessing and maintaining services and supports Communicate with families, providers, and oversight agencies Maintain accurate documentation and ensure program compliance Monitor services and address participant needs as they arise Qualifications Bachelor's degree required Experience working with individuals with disabilities, TBI, and/or older adults Knowledge of Medicaid and community-based service systems preferred Strong writing, organizational, and interpersonal skills Ability to work independently and manage a caseload Based in New York City with reliable transportation Comfortable traveling across all five boroughs and entering homes Spanish, Russian, or other language skills strongly encouraged Technical Skills Microsoft 365: Word, Excel (basic),Outlook, Teams SharePoint:
