Bronx, Brooklyn, Manhattan, Queens, and Staten Island Join Our Team at A1 Service Coordination A1 Service Coordination is a conflict-free agency supporting individuals in the Traumatic Brain Injury (TBI) and Nursing Home Transition & Diversion (NHTD) Medicaid Waiver programs. We serve participants across New York City and the Lower Hudson Valley. We're hiring a full-time Service Coordinator (SC) based in New York City to provide person-centered, community-based support to individuals with disabilities, TBI, and older adults. Important Travel Requirement This role requires regular in-person home visits across all five boroughs: Bronx, Brooklyn, Manhattan, Queens, and Staten Island. Candidates must be willing and able to travel to all boroughs as needed. This is a hybrid position combining remote work with field visits. Candidates should be comfortable using public transportation or other reliable means to travel throughout NYC and entering participants' homes. Who We're Looking For Experienced professionals who are comfortable working independently in the field and are passionate about supporting individuals in maintaining independence in the community. Spanish, Russian, or other language skills are strongly preferred.
