Job Description

Facilitates discussion Groups to clients and/or individuals currently in County Dentation Centers. Provides group and individual support services to clients. Provide "Reach-in" supports to incarcerated participants as soon as the individual has been identified. "Reach-in" is the process of building connections between the Recovery coach and incarcerated individuals to assist in the transition from jail to clinically appropriate community services. The "Reach-in" supports provided with the curriculum "Getting into the Driver's Seat of Your Treatment, Preparing for Your Plan," by J. Tondora, (Yale Program for Recovery and Community Health, 2011); Provide peer support services to engage justice-involved individuals with an OUD in a mutually empowering peer relationship to support "Re-entry" transition from incarceration to the community; Ensure that a follow-up appointment is scheduled for the participant with clinically appropriate community based services within forty-eight hours of release, in keeping with national best practices; and Provide OUD overdose prevention education prior to release Assist clients in articulating personal goals for recovery and supports individuals in identifying and creating goals and developing recovery plans with the skills, strengths, supports and resources to aid them in achieving those goals. Assist clients in setting up and sustaining self-help (mutual support) groups, as well as means of locating and joining existing groups. Model the value of every individual's recovery experience: effective coping techniques and self-help strategies. Assist in obtaining services that suit that individual's recovery needs. Assist clients in developing empowerment skills and combating stigma through self-advocacy. Works under moderate supervision with minimal latitude for the use of initiative and independent judgment.
