Responsibilities of LIDDA Service Coordinators include: Provide assessment, service planning, coordination, and monitoring of services for people on assigned caseload. Conduct activities through face-to-face and telephone contacts with individuals, providers, family members/legal guardians. Assist people on assigned caseload to establish and/or maintain eligibility for services. Promote Person Centered Thinking and self-determination by educating individualsabout rights, options, and choice. Complete and submit all required program documentation in accordance within the Local IDD Authority (LIDDA) Departments designated timeframes. Ensure all activities support and promote StarCare's culture of hope and health.
Schedule:
Full-time, non-exempt positions scheduled Monday - Friday 8am - 5pm with flexibility to meet the needs of the program.
Compensation:
$20.30/hour

Educational Requirements:
Bachelors degree from an accredited four (4) year college or university,required. Major in a Human Services or related field (Sociology, Psychology, Family Studies, etc.),preferred. One (1) year of full-time experience working with individuals with intellectual ordevelopmental disabilities,preferred.
Additional Requirements:
This position requires strong, detailed oriented writing skills. Must possess strong organizational skills, communication skills, and compassionate patience when interacting with individualson caseload. Knowledge and skill in operating personal computer with Microsoft Office products is required. Position requires frequent travel within the service delivery area using personal vehiclewith mileage reimbursement providedat the Agencys approved rate. Must have current Texas drivers license, liability car insurance, and be insurable under the Agency insurance. Center laptop computer and cell phone are provided. Must pass a pre-hire drug screen and criminal background check. Must want to ensure all people have the opportunity to live their BEST lives
