Job Description

Promotes and supports independence for participants through development, implementation and evaluation of MAPs (My Action Plans), instructional learning opportunities, and community integration support services. Works collaboratively with other Agency team members and provider resources.
CASE MANAGEMENT
Responsible for overall case management process for individuals within assigned caseload; inclusive of participant assessment process, MAP (My Action Plan) development/monitoring, staffing process, behavior support plans, ensuring documentation that is complete, correct and on-time.
CASE MANAGEMENT
Coordinates and conducts meetings reviewing individual's progress on MAPs; makes recommendations regarding program continuation documenting as appropriate and required.
INSTRUCTION
Develops, implements and monitors instructional learning opportunities with participant input focused on participant self-determination growth and development.
ACTIVITIES
Plans, develops and monitors daily activities for Day Services Programming.
DIRECT SUPPORT STAFF INPUT
Collaborates with direct support staff in the provision of participant support plans, individual goals, educational/instructional needs and activities ideas. Serves as a resource/mentor for Day Services Program staff. Bachelor's Degree in Human Services / Counseling / Rehabilitation / Social Work / Education or related field required.

Clear Criminal Background Check and Caregivers Background Check; Clear Wisconsin Motor Vehicle Record. CPR/First Aid/AED certification.

Minimum three-to-five years prior, proven direct experience with individuals with intellectual/developmental disabilities.

Excellent interpersonal and communication skills. Independent problem-solving ability. Microsoft applications (Word and Excel).
