Job Description

About Us Christian Servants Home Care, LLC is a non-medical agency serving individuals of all ages and abilities. We provide person-centered services designed to support growth, independence, and well-being. Our services include individual skill development and enhancement, employment skills training, parent mentoring, and mentoring for both adults and children. Comprehensive Community Services (CCS) is a recovery-oriented program that supports individuals in reaching their fullest potential by addressing unique mental health and substance use needs. Position Summary This position provides direct, community-based services to clients enrolled in CCS. Services focus on psychoeducation, mentorship, and skill development to support individualized recovery goals. The ideal candidate is compassionate, reliable, and experienced in implementing evidence-based practices. Primary Duties & Responsibilities Provide weekly in-home sessions (1-4 hours) focused on psychoeducation, mentorship, and skill development aligned with client goals Transport clients to and from approved activities and appointments as needed Complete daily documentation logs accurately and on time Attend regularly scheduled team meetings Participate in monthly supervision meetings with a clinical practitioner and complete supervision logs Engage in ongoing check-in meetings with management Travel to serve clients across Marathon, Lincoln, Langlade, Forest, Vilas, Oneida, Taylor, Price and Portage counties Utilize professional tools, resources, and experience to deliver skilled interventions and evidence-based practices Minimum Qualifications Applicants must meet one of the following education requirements: Bachelor's degree in human services or a related field OR Master's degree in human services or a related field And must also have: Experience working with individuals managing mental health and/or substance use challenges Ability to facilitate meaningful, proactive services that support recovery and personal growth Willingness to complete 20 hours of required online training and competency prior to service delivery Commitment to completing 8 hours of continuing education annually Additional Requirements Completion of all onboarding paperwork and required training through Christian Servants Home Care Access to a cell phone capable of clocking in/out, tracking location, and completing documentation after each visit Strong communication skills and responsiveness Reliability, compassion, and dedication to providing high-quality, person-centered care Please contact by e-mail with any questions: taylor.leonard@christianservantshomecare.com Christian Servants Homecare, LLC utilizes e-verify to verify employment eligibility.
