Job Description

Community Enagagement Specialist 2954 Buffalo Creek Rd, Chattaroy, WV 25661 $15 an hour - Full-time $15 an hour - Full-time This is a full time position requiring travel between locations, primarily in Logan and Mingo Counties in
WV. POSITION PURPOSE
This position requires extensive knowledge and understanding of mental illness and substance abuse. Position requires knowledge of community agencies and resources, the mental hygiene process and the ability to relate to patients, their families and significant others. This position encompasses both non-traditional work settings and hours.
ESSENTIAL JOB FUNCTIONS
Serve as a liaison between the patient and all other community agencies and resources. Meet the patient on their "turf" to ensure treatment compliance. Complete all reports and documents in a timely fashion. Transport consumers in MLIH rental vehicle, when needed. Perform other duties as may be assigned by immediate supervisor. Meet the weekly and/or monthly goals for consumer and community resource services and contacts. Maintain daily service log and submit weekly schedules in advance as determined by the supervisor. Present a professional demeanor and maintain professional boundaries as outlined by the supervisor at all times. Adhere to the agency policies and program requirements governing the use of provided tools, such as cell phones, vehicles, laptops, and tablets, etc. Acknowledge that this position is grant funded and future employment is dependent upon the availability of continued funding. The majority of work tasks are completed in settings other than in a traditional office. A high level of personal integrity and accurate record keeping is required.
MINIMUM QUALIFICATIONS
High School diploma or equivalent with some college preferable. Some experience preferable in a human services field.
