Full Job Description Shifts Available:
Saturday and Sunday Only•12-hour shifts; and Monday, Wednesday, Friday Only•12 hour shifts
Duration:
7 am to 7 pm; 7 pm to 7 am
Essential Job Functions :
1. Assist individuals in acquiring skills related to daily living, self-care and other needs as outlined in the habilitation plan, to afford opportunities for community living and active, valued participation. record data indicated; follow house and Day habilitation schedule. 2. Participate in the assessment and interdisciplinary team process. 3. Ensure food preparation follows prescribed menus and housekeeping standards are maintained. 4. Administer and document medication and treatments to individuals in accordance with state approved guidelines; meet annual certification guidelines for medication administration. 5. Administer first aid to individuals and report treatment to Administrative staff; maintain current certification in CPR and first aid. 6. Assist individuals in repositioning and transfer using approved lifts / equipment or 2 person transfer procedure; assist in maintaining all equipment. 7. Drive/accompany individuals to designated activities as per day habilitation schedule and other activities and events to include doctor's appointment, shopping, out to eat; assist individuals in planning/ scheduling activities. 8. Attend monthly house meetings. 9. Document unusual incidents as per agency policies and procedures. 10. Other duties related to the supports required by the individual. 11. Assist in routine household repairs and yard maintenance. 12. Ensure timey notification of necessary repairs to include those related to plumbing, electrical, cosmetic, and furniture. Requirements:
Must be 18 years old or older HS Education/ GED Must have a valid Driver's License Must pass
