Your primary focus will be to enhance the quality of life for clients by supporting their physical, emotional, and social needs in various settings, including home health and assisted living environments. Duties
  • Provide continual monitoring of program participants including A.D.L.'s
  • Providing support according to each participant's needs.
  • Coordinate and conduct transportation services as needed.
  • Provide support according to each participant's needs.
  • Escort and organize DTA indoor and community activities.
  • Develop on going variety of activities that will focus on enhancing and developing ADL's and social skills.
  • Carry out behavior modification techniques, (CIT) maintain daily logs and quarterly reports.
  • Monitor program performance in compliance with program standards.
  • Attend staff meetings and participate in program standards.
  • Plan routine practical exercise (walking, outdoor or gym exercise, etc.)
  • Assist clients in maintaining a clean environment.
  • Complete daily communication logs and tracking sheets
  • Drive company vehicle to pick and drop off members Experience Ideal candidates will possess experience in caregiving or related fields such as home health or assisted living.
Skills in memory care, cooking nutritious meals, and understanding the needs of individuals with developmental disabilities are highly valued. Familiarity with vital signs monitoring and adherence to HIPAA guidelines is essential. A compassionate demeanor, strong communication skills, and a commitment to providing high-quality care are crucial for success in this role.
