Job Description

Job Summary:
The Direct Care Worker (DCW) is responsible the health, safety and care of the Member to whom they are assigned. They are responsible for following the Member's Service Care Plan. They are respectful of the Member's property and family dynamics and they work well with Agency management and staff.
Requirements :
At least 18 years old. Ability to read, write and follow a written plan of care and document care as given. Ability to provide proof of right to work in the United States. Successfully complete the DCW training program mandated by the State of Arizona. Maintain current CPR Certificate. Ability to pass a criminal background check.
Duties and Responsibilities:
a) Member Maintain a safe, clean, and healthy working environment; including but not limited to, using universal precautions and infection control procedures at all times while working with the Member; Safely and appropriately assist the Member, as per the Service Care Plan, in personal hygiene and grooming including. Use proper body mechanics and safe techniques while positioning and/or transferring the Member; Demonstrate a good working relationship with the Member. Complete, as per the Service Care Plan, for the Member such as, laundry, vacuuming, dusting, mopping, sweeping, dishes, cleaning living areas, assist member with errands. b) Documentation Observe, Report and Document on the Member's status and whether or not the services are being provided appropriately. Complete paperwork/timesheet daily. Turn in complete and accurate paperwork/timesheet every Monday. Responsible for reporting and documenting Member concerns to appropriate staff and following up. c) Agency Comply with the Policies and Procedures of the Agency; Demonstrate a good working relationship with the office staff; Demonstrate professional work ethics and reliability; Perform work timely and efficiently; Produce Incident Reports when necessary; Maintain confidentiality when handling Member files and information; Other duties and responsibilities as assigned by supervisor;
Physical Requirements:
Ability to stoop, bend, lift, turn and walk unaided; Ability to safely transfer Consumers when needed (i.e. from bed to wheelchair).
Pay:
$15.15 per hour
