Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patients' health status by observing vital signs and reporting changes promptly Administer medications accurately following care plans and HIPAA regulations to ensure patient safety Prepare nutritious meals tailored to individual dietary needs and assist with feeding as necessary Use specialized equipment such as Hoyer lifts or wheelchairs to safely transfer clients, ensuring proper body mechanics and safety protocols Maintain detailed documentation of care provided, patient observations, and any incidents in compliance with healthcare standards Support clients with memory care needs such as Alzheimer's or dementia by implementing personalized routines and engaging activities Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe living environment Collaborate with healthcare professionals to develop and follow individualized care plans that meet each client's unique needs Provide emotional support and companionship to foster a positive and trusting relationship Skills Proven experience in home & community care, assisted living, or long-term care environments Knowledge of patient observation techniques and vital signs monitoring Certification in CPR and First Aid required; additional certifications such as HIPAA training are a plus Familiarity with working with individuals with developmental disabilities, autism, or dementia care needs Ability to perform heavy lifting safely using equipment like Hoyer lifts when necessary Strong communication skills for effective interaction with clients, families, and healthcare teams Attention to detail in medication administration, care documentation, and adherence to safety protocols Compassionate attitude combined with patience and resilience in challenging situations Experience working within hospice or home health settings is advantageous Join us in providing exceptional caregiving that enriches lives!
