In this vital role, you will provide essential assistance with daily activities, medical needs, and emotional well-being, fostering independence and dignity for those you serve. We are dedicated to creating a supportive environment where your caregiving skills are valued and your efforts directly impact quality of life. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary Monitor patient health by observing vital signs, managing medication administration, and documenting medical records accurately within Electronic Health Record (EHR) systems Support clients with specialized care such as Alzheimer's, dementia, hospice, and memory care needs by implementing tailored strategies to ensure safety and comfort Perform light housekeeping tasks including cleaning, laundry, meal preparation, and maintaining a safe living environment Use medical terminology appropriately when recording patient information and communicating with healthcare teams while adhering to HIPAA regulations for confidentiality Assist with medical procedures such as catheter care, wound care, and taking vital signs while ensuring patient safety at all times Work collaboratively within home & community care settings or assisted living facilities to deliver comprehensive patient care that promotes independence Requirements Proven experience in home health care, long-term care, nursing homes or assisted living environments with familiarity in working with individuals with disabilities or developmental challenges Knowledge of medical terminology, medication administration protocols, and documentation practices using EMR/EHR systems Certifications in CPR and first aid are required; additional certifications such as hospice care or dementia training are preferred Ability to lift patients safely using equipment like Hoyer lifts and perform physically demanding tasks including assisting with transfers and mobility support Strong observational skills for patient monitoring and the ability to respond promptly to emergencies or changes in condition Excellent communication skills to effectively collaborate with healthcare professionals, patients, and family members while maintaining HIPAA compliance Flexibility to work various shifts including live-in arrangements or weekends as needed; a caring attitude combined with patience and empathy is essential Join us in providing exceptional home & community care that enriches lives!
