This role involves delivering personalized care that promotes independence, safety, and well-being for clients with diverse needs, including seniors, individuals with disabilities, and those requiring specialized care such as Alzheimer's or dementia. The ideal candidate will possess a strong background in patient observation, medication assistance, and activities of daily living (ADLs), ensuring a respectful and nurturing environment. Responsibilities Assist clients with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary. Monitor patients' health status by observing vital signs, reporting changes, and maintaining detailed care records in compliance with HIPAA regulations. Provide medication reminders and assist with medication administration under supervision or according to care plans. Prepare nutritious meals tailored to dietary restrictions and preferences while maintaining proper food safety standards. Support clients with housekeeping tasks including cleaning, laundry, and maintaining a safe living environment. Implement individualized care plans designed by healthcare professionals to meet each client's unique needs. Engage clients in social activities and companionship to promote mental stimulation and emotional well-being. Maintain thorough documentation of patient progress, incidents, and observations for team communication and ongoing care adjustments. Experience Prior experience working as a caregiver or home health aide in settings such as assisted living facilities, nursing homes, or group homes is highly desirable. Demonstrated knowledge of patient care techniques including assisting with ADLs, medication management, and vital signs monitoring. Familiarity with specialized care such as Alzheimer's/dementia care, hospice support, or working with individuals with developmental disabilities or autism spectrum disorder. Experience operating medical equipment like Hoyer lifts or other mobility aids is advantageous. Knowledge of HIPAA regulations ensuring confidentiality and privacy of patient information. Proven ability to handle heavy lifting safely while supporting residents' mobility needs. Strong interpersonal skills to work effectively with diverse populations including seniors and individuals with disabilities.
