Compensation:

Responsibilities:

Develop and oversee individualized care plans tailored to each client's unique needs, ensuring they receive the highest quality of care. Collaborate closely with healthcare providers and family members to coordinate comprehensive care strategies that enhance client well-being. Monitor client progress regularly, adjusting care plans as necessary to reflect changes in health status or personal preferences. Provide compassionate support and guidance to clients and their families, fostering a trusting and supportive environment. Ensure all care activities comply with industry standards and regulations, maintaining the utmost professionalism and integrity. Facilitate effective communication between clients, caregivers, and healthcare professionals to streamline care delivery.
