JOB SUMMARY
The Relative Support Worker (RSW) provides direct support to caregivers participating in the Relative Support Services (RSS) program. This role is responsible for engaging caregivers within 48 hours of referral, assessing family needs, and connecting clients to appropriate services and resources. The RSW works closely with Relative/NREFM caregivers who are providing care for children involved with the Department of Children and Family Services (DCFS), ensuring timely service delivery, advocacy, and compliance with program and grant requirements. This position requires strong case management skills, the ability to navigate sensitive family situations, and a commitment to supporting at-risk populations.KEY RESPONSIBILITIES
Client Engagement and Case Management Contact caregivers within 48 hours of referral to initiate services Conduct comprehensive needs and strengths assessments for families and caregivers Develop and implement individualized service plans aligned with client goals Provide ongoing case management, support, and advocacy for caregivers Educate clients on available voluntary services, rights, and mandated reporting requirements Service Coordination and Documentation Coordinate referrals to community resources, including kinship support services Maintain accurate and timely documentation of all client interactions within 24 hours Ensure all case files meet program, funder, and regulatory compliance standards Track participation and progress toward service goals in accordance with grant requirements Outreach and Engagement Conduct outreach to ensure participation and retention in services Build relationships with community partners and service providers Support caregiver engagement through consistent follow-up and communication Collaboration and Program Support Work closely with Program Manager and internal teams to support service delivery Participate in team meetings, case reviews, and training as required Contribute to program quality improvement efforts and compliance initiativesKNOWLEDGE, SKILLS & ABILITIES
Knowledge of family support services, early childhood development, and community resource systems Ability to build trusting relationships with families from diverse backgrounds Strong communication, interpersonal, and relationship-building skills Ability to assess family needs and provide appropriate referrals Strong organizational and documentation skills Ability to manage multiple priorities and meet deadlines Cultural humility and experience working with diverse populations Proficiency in Microsoft Office and data entry systems Ability to work independently and as part of a multidisciplinary team Ability to maintain confidentialityTRAVEL REQUIREMENTS
Local travel within the Antelope Valley region required to support services and outreach Occasional travel to off-site trainings, meetings, or agency events Mileage reimbursement provided in accordance with AVPH policyREQUIRED QUALIFICATIONS
Completion of a minimum of 32 semester units from an accredited college or university in Social Work, Social Sciences, Humanities, Public Administration, Public Health, or a closely related field Two (2) years of professional experience in the field of social work, behavioral science, or social science Experience working with children, families, or community-based services Experience connecting individuals or families with community resources Strong interpersonal and communication skills Ability to work with culturally diverse populations Valid California driver's license, reliable transportation, and proof of current automobile insurance required for business-related travel Bilingual (Spanish or other languages reflective of the community served) preferred Annual TB test requiredPREFERRED QUALIFICATIONS
Associate's or Bachelor's degree in Social Work, Public Health, Psychology, Child Development, or related field Experience working in family support programs, kinship care, or early childhood services Experience working with high-risk or vulnerable populations Experience using case management or data systems Experience with audits, monitoring, or quality assurance processesADDITIONAL INFORMATION
This position is grant-funded with a projected end date in December 2026 and contingent upon continued funding and program needs. Must maintain regular and punctual attendance in accordance with agency standards. Must comply with all AVPH policies, procedures, and regulatory requirements. Job duties may be modified to remain compliant with programmatic or grant-related changes.CORE COMPETENCIES
