In this vital role, you will provide attentive support to clients in their homes, assisting with daily activities and ensuring their comfort, safety, and well-being. Your energy and empathy will help foster independence and dignity for those you serve, creating a positive impact every day. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support. Monitor patient health by observing vital signs and reporting changes promptly. Support medication reminder following proper protocols and documentation standards. Provide companionship and engage clients in meaningful activities to promote mental stimulation and emotional well-being. Maintain accurate medical records using Electronic Health Record (EHR) systems while ensuring compliance with HIPAA regulations. Perform light housekeeping tasks such as cleaning, laundry, and meal preparation to support a safe and comfortable environment. Utilize specialized equipment like Hoyer lifts or catheters safely to assist with mobility and hygiene needs. Requirements Prior experience in home health, assisted living, nursing homes, or long-term care settings preferred. Knowledge of medical terminology, hospice care, dementia/dementia care, Alzheimer's care, and memory care best practices. Certifications such as CPR and first aid are highly desirable. Ability to work independently while following detailed care plans and medical documentation procedures. Experience working with individuals with developmental disabilities or disabilities is a plus. Strong patient observation skills to identify changes in condition or behavior promptly. Valid driver's license and reliable transportation preferred for home visits.
