Responsibilities Assist clients with activities of daily living such as bathing, dressing, grooming, and mobility support Provide companionship and emotional support to promote mental well-being Administer medication reminders and assist with light housekeeping tasks as needed Monitor clients' health status and report any changes or concerns promptly Follow care plans accurately and document all care provided in detail Support clients in maintaining their independence and quality of life in a safe environment Communicate effectively with family members and healthcare professionals to ensure coordinated care Skills Compassionate and patient demeanor with a genuine desire to help others Strong communication skills for effective interaction with clients and team members Ability to follow detailed care instructions and documentation procedures Experience working with diverse populations, including geriatrics or individuals with chronic conditions Knowledge of basic medical procedures, including medication management and emergency response protocols Prior experience in home health, post-acute care, or outpatient settings is highly valued Ability to adapt quickly to changing client needs while maintaining professionalism Join us in making a meaningful difference every day!
