Duties Deliver personalized care by assisting with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts and catheter care when necessary Monitor patient health through observation of vital signs and overall condition, promptly reporting changes to healthcare professionals Administer medications accurately following medical documentation protocols while maintaining strict adherence to HIPAA regulations for patient privacy Support patients with dementia or Alzheimer's by implementing specialized memory care techniques and engaging them in meaningful activities Maintain detailed medical records using Electronic Health Record (EHR) systems and document all care provided with precision and professionalism Assist with meal preparation, feeding, laundry, cleaning, and maintaining a safe home environment to promote comfort and hygiene Provide emotional support and companionship to foster trust and enhance quality of life in home or community settings Experience Prior experience working in assisted living facilities, nursing homes, or home health environments is highly desirable Demonstrated knowledge of medical terminology, medication administration, hospice care, and long-term care practices Hands-on experience with patient observation, dementia care, Alzheimer's care, and working with individuals with developmental disabilities or disabilities in general Familiarity with EMR (Electronic Medical Records) systems and EHR (Electronic Health Records) documentation standards Certification in CPR (Cardiopulmonary Resuscitation) required; additional certifications such as hospice training or specialized caregiving courses are advantageous Proven ability to perform caregiving tasks including vital signs measurement, catheter care, use of Hoyer lifts, and working safely with medical equipment like wheelchairs or mobility aids Strong communication skills for working effectively with patients, families, and healthcare teams in home & community care settings Join us if you're passionate about providing compassionate care that truly transforms lives!
