This rewarding position offers the opportunity to provide personalized, respectful, and professional care in clients' homes or community settings. As a Home Care Aide, you will be an essential part of a dedicated team committed to promoting independence, dignity, and well-being for those we serve. Your energy and empathy will make a meaningful difference every day. Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary. Maintaining a clean, safe, and healthy environment within the client's residence. The performance of all personal care activities contained in a written Plan of Care Monitor patient health by observing and recognizing changes in condition, and reporting findings accurately using Assisting Hands system. Support clients that could include specialized needs including Alzheimer's care, dementia care, hospice care, etc. Engage in home & community care activities that support clients' independence while respecting their privacy and preferences. Abide by policies and procedures of Assisting Hands Santa Clara and CDSS and HIPAA guidelines
Hours:
Dependent on assignment; Shifts available 24 hours a day, 7 days per week and based on availability. Ideal Experience Prior 1+ year experience in home health care environment. Understanding of/Willingness to learn medical terminology relevant to patient care scenarios. Proven ability to handle sensitive situations with compassion while maintaining professionalism Reliability on meeting commitments and open communicator
Pay:
$22.00 - $24.00 per hour
