Compensation and benefits:

Key Responsibilities:

Develop, implement, and manage individualized care plans that ensure the highest quality of in-home care Build and maintain strong relationships with clients, families, caregivers, and referral partners Serve as the primary point of contact to ensure care is safe, appropriate, and aligned with client needs Support and mentor caregivers to promote excellence in care delivery Act as a liaison within the local community to strengthen partnerships and expand awareness Maintain clear, consistent communication with all stakeholders Balance multiple priorities while maintaining a calm, positive, and solution-oriented approach

Qualifications:

High school diploma or equivalent required; additional education or training is a plus Minimum of 2 years of professional experience in a related field Valid California driver's license Reliable personal vehicle for in-home visits throughout San Luis Obispo County (mileage reimbursed) Availability for occasional on-call responsibilities outside normal business hours
