Job Description

Introduction:
Family Ties Home Care has a client in Simi Valley who needs night time help so his wife can get her sleep. Client is in his 70's with dementia and is a fall risk. He needs to be watched and assisted when he gets up at night to use the restroom.

Responsibilities:
Following a pre-set care plan Standard ADLs (personal care, bathing, dressing, grooming, hygiene assistance) Assist with medication reminders Monitor and report on client's well-being Light housekeeping, laundry, & other household daily chores as needed Grocery shopping & errands Cooking/meal preparation Scheduling appointments, including doctor's Communication with Family Ties HC management & family members/loved-ones regarding the client's daily status Daily notes Other duties assigned as needed
Qualifications:
Current HCA or higher certification Caregiving experience of 1 year or similar experience Compassionate and caring demeanor Excellent communication & interpersonal skills Ability to follow the pre-set care plan and medication schedule Ability to lift 35 pounds Ability to assist with client's mobility as needed Valid driver's license and reliable transportation Ability to work the schedule as described Physical Demands and Work Environment The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this position.
