Overview Join our compassionate healthcare team as a Certified Home Health Aide (CHHA), where you will play an essential role in supporting patients in the comfort of their own homes. As a CHHA, you will provide hands-on personal care, emotional support, and companionship while promoting patient dignity, independence, and overall well-being. Duties Assist patients with activities of daily living (ADLs), including bathing, grooming, dressing, toileting, and personal hygiene. Help patients with mobility, transfers, ambulation, and range-of-motion exercises as directed by the care plan. Provide companionship and emotional support to patients and their families. Assist with meal preparation, feeding, and monitoring nutritional intake according to dietary guidelines. Perform light housekeeping duties, including laundry, changing bed linens, and maintaining a safe, clean environment. Observe and report changes in patients' physical, emotional, or mental condition to the supervising nurse or healthcare team. Take and record basic vital signs, such as temperature, pulse, respiration, and blood pressure, as permitted by state regulations and agency policy. Remind patients to take medications as directed by their healthcare provider, in accordance with agency policies and state regulations. Assist patients with transportation to medical appointments, errands, or community activities when appropriate. Support patients with memory care needs, including those living with dementia or Alzheimer's disease, through compassionate and patient-centered care. Maintain accurate documentation of services provided and patient observations in accordance with agency policies and HIPAA regulations. Collaborate with nurses, therapists, physicians, and family members to support the patient's individualized plan of care. Promote patient safety by identifying potential hazards and implementing fall prevention measures. Qualifications & Experience Current Certified Home Health Aide (CHHA) certification required in accordance with state regulations. Previous experience in home health, caregiving, assisted living, skilled nursing facilities, hospice, or related healthcare settings preferred. Experience working with geriatric patients, individuals with disabilities, or those with chronic illnesses is highly desirable. Knowledge of infection control practices and basic safety procedures. Strong communication and interpersonal skills with the ability to build trusting relationships with patients and families. Ability to work independently while following established care plans and reporting requirements. Compassionate, reliable, and committed to providing high-quality patient care. Ability to maintain confidentiality and comply with HIPAA regulations. Current CPR certification preferred. Valid driver's license, reliable transportation, and proof of insurance may be required depending on assignment needs.
